CoSteps Click here to see/print a sample form. The patient population encountered in home health is diverse and they usually have more than one condition. The CoSteps are diagnosis–specific adjuncts to the primary diagnosis Home Care Steps® Protocol. CoSteps are indicated only when the clinician anticipates the secondary condition to impede progress through the primary pathway. Goal: These are episodic goals for the secondary condition. Outcomes: The CoStep includes condition-specific outcomes that are in addition to the outcomes on the primary diagnosis pathway being used. It is set up as a multiple visit note. It fits five visits of documentation. Therefore, if more visits are anticipated, then more than one of the same CoSteps is placed in the chart to be available for documentation later in the episode. When outcomes from the CoStep are addressed and evaluated, then they are documented as met with a "check" or as not met with a "variance code." Usually, agencies require documentation of at least one outcome on every visit (beyond the first or second visit). If the CoStep outcomes are not met by the end of the episode of care, then this may provide evidence for need for continued services. The primary diagnosis may then change to the diagnosis used for the CoStep. |
