Agencies need to provide for efficient CASE
MANAGEMENT strategies. One way this may be
promoted is by using the Home Care Steps®
Tracking Tool or the Variance Close-Up on a
visit-by-visit basis. These tools allow for an
at-a-glance look at how the patient is doing
along the pathway in the midst of all of the
paper. This one page document will allow you to
visualize how often the patient status was
outside normal limits (Outcome #1 on every
visit), how often the patient used unplanned
health care resources outside of home care
(Outcome #2 on every visit), how often
unscheduled visits were made, how many visits
were made, how often visit protocols were
repeated, and the variances for why outcomes were
not met. Also, it allows the clinician providing
the visit with an efficient way of identifying
unmet outcomes from previous visits that must be
addressed. Further, if the agency has been
allowed a certain number of visits, the clinician
can more efficiently identify if communication is
need to the payer because outcomes aren't being
met as anticipated. This must be done before the
last visit to more successfully get approval for
additional needed visits. |
